Administration Fees  Due!

Annual fees have not been collected for 2009.

Notices were sent out and if we have not received your annual fees by JANUARY31ST,  2009, which are $200.00 per year based on amount of participants in your classes.  

If you did not get your notice then we do not have current information such as mailing address, phone number.

Flat fees are $550.00 unlimited.

$250.00 under 200 participants.

$150.00 under 100 participants.

New Rules

We have allowed certificates in the pass to unnumbered in order help speed up time and efficiency of delivery, however, effective May 1, 2009 ALL certificates will have to purchase under our old system.  

Administrative Fee

Each certificate will cost $5.00 each per car per course per participant.  Example, CPR-First Aid is $10.00 administrative fee for cards.  All cards must have sequential numbering.  All cards must have agency  name, location, city, state and zip with phone number. Each agency is designated  a region code established by us and this must be included on the cards.  

Equipment Rental

If you rent equipment from us, shipping or delivery is based on current UPS charges or IRS mileage reimbursement fee schedule.  Return of equipment is paid by agency requesting.  We will not rent out more than 30 manikins to any one agency at one time.

Course length

Course length is based upon your discretion to determine number of participants-equipment ration.  

Course Material    

Many of the agencies have taken upon themselves to alter teaching information we sent out.  Not Agency Instructor books,  this information is the basic information you teach with.  If you added or deleted any information you have to send us a copy for our records and we can place this in your file as to the core information your instructing.  This is important to review.  It needs to meet current standards guidelines and many OSHA, MSHA, Maritime requirements.  We will notify you if your information does not meet the current standard recommendations.   

Flexibility

Many regions have certain guidelines they think are important to maintain there structure.  We have no problem with these amendments but be sure to send us a copy.

Instructor agency 

Agencies are not limited to the amount of instructor they can have in any one region.  They must adhere to guidelines established by the agency instructor trainer whom has been given guidelines by us.

Final status approval.

We have mailed out nationally for all regions CPS, DPRS, county agencies of the certificate changes and agency status.   All agencies must submit a current list of instructors, we do not need ssn, address and phone numbers for these individuals.  This is your responsibility should we call to request this information in cases of wrong information being instructed or students complaints.

Please include a copy of your present authorization certificates or licenses with a current photo (2) for identification purposes. One will be placed on your identification card to be mailed out to you. Certain fees are involved depending on your status request.

Once your information had been received and reviewed by our Agency Committee you will be notified by either email or written notification.

*Master Instructor - 10 years plus or 50 classes

*Master Trainer - Must have taught over 75 classes or 1000 participants.

* Instructor    Must teach 10 classes per year or 100 participants
